Description

This is a front and back page designed to ease the recording of specific information used to grade students on a daily basis. It is a set of 8 circles with a code for what each circle represents. There are 8 circles for each day of the week for a 6 week recording period. The code can be adjusted to suit your needs. I recommend making a copy before editing, it may take a couple tries to get it the way you want. The bottom half is for student schedule, and the back is for recording contacts and observations.
A student secretary could also easily record information, or a few students could record information for a small group for a set amount of time and submit the reports at the end of each class for their group. So one student could be responsible for tracking five students in a week. The student in charge could change each week/month, etc.
The best way to check off bubbles is with a yellow thick crayola, it is quick and you can see the code letter through the ink.
