This Data book has forms for inclusion in a data binder. These forms are for teachers required to implement CFA's- Common Formative Assessments within their classrooms.

What is a CFA?
(Common Formative Assessment)
•Formative Assessment is any assessment (task + tool) that gives the teachers and/or students feedback about learning.
•If it gives the teacher information to use to guide the instruction of the students assessed, it’s formative.
•If it gives the student information that guides learning, it’s formative.
•If it tells the teacher what to do differently or next, it’s formative.
A Common Formative Assessment includes a clearly articulated assessment task, calibrated evaluation tool (teachers must be in agreement in how the assessment will be scored), and a plan for administration and scoring.
