As a Special Education teacher. every year I make a new set of data collection sheets and this year is no exception. This is a user friendly template that gives a snapshot of each of your students. I fill in the pertinent info, including any medical alerts, add a photo and put these in my sub plans as well as give a copy to my Para Professional and my inclusion teachers.
