Description

Use this document to keep in the front of my data binder. This spreadsheet will make meetings, conferences, and data chats a breeze with the information at your fingertips.
I used this data collection form to record SRI levels, standardized test levels, and running record levels. The columns can be edited for any other tool that you use to measure growth.

How to use:

Record the names of the students down the left hand column of the document.

Fill in the data from the previous year if known. For example, I would fill in 2nd grade SRI scores, Diagnostics, and Running Record levels.

The ‘F’ column is where to record the beginning of the school year, or fall data.

The ‘W’ column is for the winter.

The ‘S’ is for the spring or end of the year.

*The Running Record section- The ‘Ins.’ is for instructional levels and the ‘Ind.’ is for independent levels. The box with the diagonal line is for the comprehension score and the writing score.
