Each document is designed the same for student familiarity. There is an area for name and date, goal, area of concentration for goal, goal finish date, 4-3-2-1 graph for self reflective shading, date slots for each conferring time, and 4-3-2-1 scale key.

These sheets for great for data notebooks, as well as tracking data, parent communication, and conferring with students. This is self reflective so it is a student document meant for student use and evaluation.
