Here is a wonderful freebie that you can utilize in any grade in any class. Simply open the document, and type in the students name, quarter, and goal you are tracking and print!

Put the date on the top of the row and initials of who is assessing student. This is also very easy for an aide or assistant to fill out as well.

Start tracking data from the bottom. Give a "+" for a successful trial and an "-" for a missed trial for a given 10 trials.

Count how many correct trials there were, and put a dot or highlight that corresponding box to the left. Over time, this will give you a line graph to show student improvement.
