Description

This is the second in a series of 3 data collection templates. This sheet is for reporting accuracy by question or category and the total score of the assessment. You can easily create graphs that show the percentage of students at different proficiency levels.

How to use:

  • Type in student names
  • Type in codes for student accuracy
  • Type in scores
  • Calculate percents
  • Count accuracy per question
  • Use the drop-down menu to mark intervention needs

The sheet is not set to automatically calculate their percentage, but the column is color-coded by proficiency level.

To calculate their percentage, type: = number correct/total number*100

All use conditional formatting on Google Sheets to make the data quick and easy to use. If you want to change colors or proficiency levels: highlight the column(s), click Format, select conditional formatting, and click the different colors to adjust the settings.
