This product includes 2 separate products that work together to create the ultimate resource to help with your Section 3 and 6 of IEPs! Send this to students on your caseload with Executive Functioning goals to see what they consider their strengths and weaknesses. Once the form is completed, the student's answers will automatically upload to the provided Google Sheet. The answers are calculated into numerical values where a "score" is given for each section. Once all sections are calculated, a rank is created on the Google Sheet to see how each section compares!
In this resource, you will be provided:
- A Google Form Questionnaire with 12 sections of Executive Functioning
- A Google Sheet FULLY formulated for quick calculation
- A Directions sheets with links to the two products above AND explanation on how to link the two smoothly!
PLEASE NOTE: You CANNOT delete any questions or lines in the Form or Sheet. This is meticulously formualted so even one row/column being deleted will throw off the entire sheet. Please email me or message me on TPT with any questions!
