This is a Google Form Teacher Check-In, allowing you to maintain your pastoral role while distance learning takes place due to coronavirus. The form can be used to help students reflect on their own feelings, promote mindfulness, or act as a simple and confidential way for them to raise any concern with you, their teacher. The form is carefully designed to place equal weight on celebrating positives as well as reviewing possible negatives.

This can act as a great way to keep in contact with your students and continue to promote and monitor the social, emotional, and mental health and wellbeing. It is editable so can be adapted to any age group.

This is a GOOGLE FORM document; TPT requires that you give them access to copy the file over to your Google Drive – my suggestion, though you are by no means obliged, would be to copy it onto a personal Google Drive and then you can copy it across yourself to your school Google Drive.
