Have you ever had a teacher send a student down for dress code and you aren't really sure why? Do you ever have a student that you feel is a "repeat offender" for the same issue? Do you just need an easy solution for the pesky task of dress code? Then this is the form for you!

This form can be filled out by a teacher and goes with the student to the office. There is a space on the form to indicate how it was resolved and signed off on. If the student is fine, they can carry for the form with them for the day in case they are mistakenly stopped again. If there is an issue that needs to be resolved, the form can be documented and filed.
