This EDITABLE Behavior Chart can be used to document children that have IEP's or need RTI data. Make sure to edit the name, date, and behavior to be documented before using!
Discuss this chart with families for successful implementation. Come up with a reward for students who earn a set number of points in the week!
Let the student be in on the reward choice!
HOW TO USE
- Have the student keep this in their folder! Especially for students working on responsibility. Other ways to display and document on the chart could be keeping it on their desk, teacher's desk, or on a clipboard the teacher carries around.
- Keep track of the behavior as it occurs during the lesson/class by crossing off a number starting at 0 in the behavior column or circle the number of times it occurred at the end of a lesson/class.
- Have students color in the face at the end of each day to show progress!
- Together at the end of the week, discuss the number of points earned and what positive and encouraging behaviors you noticed throughout the week.
- In the comments section, write in at least one positive comment about the student!
- Parents can sign and return the behavior chart, or you can delete the parent signature section.
