When you are unable to reach parents by phone or can't speak their language, grab this easy check off form in English and Spanish! Just check off when, where, what happened, how the child felt, and what happened later in the day. It is also great to use at the end of the day when you don't have time to write out a long note! The pdf file contains two versions: English/Spanish, and English only. I have used this when teaching elementary students, so that is what it is best used for (but it may work in some older special ed classes).

Use it in these situations and more:

✸Use in person or when making a phone call

✸Give to bilingual colleague who is contacting parents for you

✸Send home to those parents who always have their phones disconnected, are always unavailable or have used all their minutes.

✸Send home with kids who walk home or ride the bus

Instructions on how to use it are included. Here's an example of what you might check:

When: Reading

Where: In class

Your child was: frustrated, mad

What happened: Student banging table, teacher couldn't understand what the child was saying.

By the end of the day:

Child was able to work through anger and start working again later.

Teacher was unable to reach parent, please call me.

Please talk to your child about this.

I have also added another form, which is shorter and more open ended so you can add your own message. This short form also comes in English only, and English/Spanish. There are three short forms to a page.
