You can use this document to list your students and the accommodations that they receive. I use it as a "quick reference" instead of looking through students' IEPs and 504 plans. It's in a spreadsheet so you can edit it with your students and print it. You can check off the accommodations that your students have or type in.

I included the most common accommodations that I've found in student IEPs/504 plans, but you can easily edit to input any other accommodations you find necessary.
