Description

This is a chart to help students regulate their own behaviour with the use of rewards throughout the day. It also helps you to keep a record of the student's behaviour to pass on to caregivers.

How I used it:

I printed this chart front and back and put enough sheets for the week into a folder/duotang. The rewards I put in place were stickers, iPad time and behaviour certificates but these can be easily changed on the form to suit your own preferences. Each morning I would sit down with the student and she would come up with her own goals for the day. At the end of each time of day (also editable) I would review the notes with her and see if she could check off that reward and receive it (iPad time was once before lunch and once at the end of the day if all three boxes were checked off). When discussing behaviour with mum, it was helpful to have notes to refer to (especially if the student taught by other teachers). I started off sending home the folder every day and then roughly once a week after a while of using it.
