Description
This is the word version of the form I leave in my student's folder to share what we have worked on during our session with their family. I generally make double sided copies so that I can get 8 weeks to a sheet. It is very simple and indicates what general skills we worked on (articulation, following directions, social skills, language, fluency) with a small space for a description. I have a comment section for both SLP and parents that way I can write down percentages or a quick note and they can jot a quick question. I also added a line to indicate whether or not home practice has been assigned, that way there is no question about it! I have found this form to be the perfect balance between giving parents some info about the session and not needing to spend oodles of time writing notes. I try to explain to parents at our initial IEP meetings that I can either spend time writing a novel about the session or actually having a session (and I prefer the later). I hope this helps make your sessions more efficient!
