With this data sheet, students can track their own progress giving them ownership for their own education and learning. You can edit this to fit your class.

This is how I use it:

- I would have students take a readiness assessment before each unit and make a note of that.

- Throughout the unit, students will track their progress through formative assessments. Ideally, you want to keep this in a folder with all of the assessments. Students should be able to access this folder on their own.

- At the end of the unit, students will record their summative score.

- Students can use the graph to see their progress through the units.

The graph has 3 lines that show when a student is below, at, or exceeding standards.
