Here is a simple and effective sheet for weekly warm ups. As long as you keep your warm up short and sweet, this should work like a charm.
This .docx version (I also have an un-editable pdf version for sale that is cheaper) is set up for a warm up every day for a week, with a place for the date under the day initial on the left as well as a place for points and corrector's initials on the right. On the last day of the week, students tally up their totals and write them at the bottom. On this pdf version, I have a scale from 1 to 5 that I give students that they circle based on their points for that week. That circled 1 to 5 is their warm up score for the week. As this version is editable, you can easily click in the footer and delete the 1-5 scale or remove the place for a corrector's initials as you see fit.
If your students are responsible enough and you'd like to save paper and be green, make this double sided and collect them every other week. It makes grading warm ups a breeze. You just look at the circled number on each side!
