This simple, half-sheet, daily report works well for students that have difficulty with other class-wide behavior systems and can be tailored to the child's individual goals. I keep it on the student's desk and let them fill it out as the day progresses to build accountability for their behavior, then send home each day. Hope this helps you in your classroom!
