The information in this document was adapted from the Mindset Works Brainology for Schools rubric. I created it for a more quantitative approach to the information in the rubric, using more student-friendly language.
The questionnaire is in both English and Spanish.

Love this resource when working with students to better understand their executive functioning skills and effort/motivation.
