This All-in1 data sheet provides space for students to segment sounds heard in words of up to 5 sounds using colored blocks/chips in Elkonin Squares. It then provides space for students to encode the words heard. You can record the student objective (ex: Segment and Encode CVC, Encode CCVC, etc.), the target patterns (ex: initial s-blends), and the percent accuracy and support. This is a reproducible and works well with many highly structured reading programs. After the student finishes, stick it in your student file and there is your data!
