As we near the testing window for the year, I want to make sure all of my students are as prepared as possible.
I created this document to use for small group instruction/review. The targets start at 5 because the lessons prior were ones I didn't think would need much in the way of practice. Write your student's names at the top of the sheet and score or check them off as you verify they have mastered each target. This is an easy data collection sheet that can be shared with your principal as evidence that you are individualizing the needs of your students by identifying their weaknesses and addressing them in small groups.
I will be creating another sheet for all of Module 2 as well if you find this one helpful.
