This is a generic module recording sheet for instructors who teach EngageNY math grades K-8. It includes boxes for numbers of proficient students, basic, minimal, and places for retests. It also includes a place to record overall scores on exit tickets. The sheet also allows teachers to enter notes about plans for non-proficient students, and request support from a school coach. This is not an individual gradebook, but an overall class record keeping sheet. As an academic coach, I made folders for each teacher, and am having them fill out a sheet as they go through each module. It helps me keep track of data school-wide and serves as a discussion point at PLC meetings.
