Ensembles field multiple requests every year to perform at various community events. Use this form to keep the lines of communication open between your community members requesting your ensembles to perform. This form is fully customizable and acts as a way to better understand what is being requested as well as what will be available to you once you are at the performance venue. Also, a section of the form will remind the person making the request that it is nice to feed and/or donate to your organization.
