This is a document that I've used with my students this year to allow them to do test corrections. In order to get credit, they must explain why they got the question wrong, show the work that proves the correct answer, and then write what important rules/concepts in math they need to remember when answering a similar question in the future.

It's pretty much an amalgamation of several other resources/procedures I've used in the past. I found that having it laid out in table format like this forces the students to stay a bit more organized, making it easier to grade.
