Checking off your to-do list for a student evaluation feels great! This two-page document provides a reference list for you to use when conducting an evaluation. Print a copy for each student you are evaluating and keep track of what you need to do and what you have accomplished. Once the assessment plan is developed, highlight all areas that need to be completed. This will create an easy visual for your student assessment plan (see preview picture for an example).

The focus of this document is for psychologists who conduct assessments for cognitive, academic, behavior, and adaptive needs.
