This resource is an editable resource created in Excel which provides both teachers, SEND workers, support staff and parents a pro forma to identify potential behaviours which may put students at risk of injury to themselves or others, identify the context and highlight the elimination or control measures that need to be put in place to resolve the issue.

This resource has been used extensively through my years of teaching (Over 12+ years) and never fails to be a clear record of the behaviours and triggers that some students with Autism and Aspergers Syndrome exhibit.

This is just another resource aimed at collecting and recording data and information on stressors and triggers to behaviour to ensure students with such behavioural needs are having their needs met as best as possible in an educational setting.
