Exit Slips and Tickets Out the Door are a great way to bring closure to a lesson. They can be used as a formal or informal mode of assessment. These work great for teachers of upper elementary, middle, and high school students who team teach or block. Just before students leave your class, hand them an exit slip or ticket and give them directions for filling them out.

4 years later... found it! So simple yet so useful. Thank you so much for both of these formative assessment forms. As a 4th grade I will mostly use the first one but at end of day or before lunch, the actual exit slip will be great and quick.
