These Extra Credit Sign In Forms will help you quickly get extra credit to students for attending a live performance! Extra credit is great before breaks, and incentivizing students to come to your show (theatre, choir, band, dance & more) can help you fundraise! Bright colors get student attention, while black & white save ink. Customize templates to each class by writing your name (& any other participating teacher’s names) with class periods/sections to easily sort through post-show to add extra credit to the grade book. Students sign their first & last name & must sign out with either their initials or signature.
