This is something I had to use already on the second day of school with my older elementary students. It's something quick I whipped up and there are two slips per page. I use these slips at a "behavior station" where students are moved to in order to refocus on how they should be behaving in the classroom. Not just for music teachers. This could work in any classroom.
There are three simple questions for students to answer:
1. What were you doing that resulted in this slip?
2. What should you have been doing instead?
3. Why is it important to model good behavior?
