This form (in both paper and digital form here) includes a section for Behavior comments, Academic and Homework progress, as well as comments for all subjects. Since this is editable, you can add or subtract any sections to better suit your needs.
I have used the form for years in my classroom of 34 5th graders. I find that I am able to get them done, using this form, in about 20 minutes. Parents are never left guessing how their child is doing in class with this easy to use weekly report form!
