This goes well with my "MIA Request" form! While schools are moving to 1:1, not every student always has easy access to grades at home and frequently forget/lose passwords when they are at school at access grades. This is a great little form (4 per page) to let students know they are failing/close to failing as a grading period ends ! I fill them out for all my classes, make a copy for the kids and keep the originals for my records.
