This document is meant to serve as a way for teachers to collect data on student performance of a student learning objective (SLO). The teacher will list the SLO at the top and indicate the performance ability of the student (meets, partially meets, and does not meet). The teacher has a place to write notes or examples of student responses in the final column labeled, "Teacher Comments." This file is useful for documentation of student learning and for sharing information during parent/teacher conferences.
