This two-step word problem (3rd grade) assessment /review packet is divided into sets. Each set has the following pages:

a) Questionnaire

b) Coloring Page and a list of answers to choose from (ie. Letters A to J)

c) Answer Key, with the mystery picture

To use, just print and distribute the questionnaires. You may opt to give your students the coloring page at the same time, or delay its distribution after they have worked on the questionnaire (ex. give it to early finishers, or distribute after a certain a time limit, or as homework).
