The self-perception surveys will help you understand how your students' mindset when learning that particular subject. You will learn their confidence level and their limiting beliefs. This information is helpful for learning which students need more encouragement.

Each survey includes differentiated options like:

  • Color the emoji that matches your feeling.
  • Draw a face that shows your feelings.
  • Rate the statement on the scale.

The self-perception surveys include:

  • Color the stoplight that best matches your opinion.
  • I wish my teacher knew... writing prompt.

Interest inventory includes:

  • 14 short questions
  • 5 long answer questions
  • Primary or wide-ruled options

When you give these surveys throughout the year, you will notice how you've encouraged and motivated your students. These are extremely helpful! We keep these in our student portfolios.

I gave my students the interest surveys the first week of school to gradually complete. The language is elementary-aged kid friendly. I only had a couple of questions from students. The smiley faces were very helpful, especially for my SPED and ESOL students.

This file was amazing! I used these to complete a class assignment during my student teaching and feel it's a great resource to use for parent-teacher conferences. This file can be used for upper elementary students, there is portions for writing or just filling in the face. Depending on your students levels, this file is perfect.
