Description
I created this set of exit tickets for my fifth graders, to use as a supplement for the GO Math program. I used them at the end of lessons for formative assessment. They can be used as exit tickets, entrance tickets, extra practice, review before assessments or any way you choose. There are six tickets on each sheet for copying purposes, I had to made only four copies. I would copy the whole unit of exit tickets for management.
The standards are printed at the top of the sheet for the teacher and the GM lesson is on the top of each student GM ticket.
I created 105 exit tickets for the year. All the common core standards are addressed many times. There is an answer key included at the end of each unit. (not included in page total)
There is a variety of types of tickets for testing prep. There are some multiple choice, true/false, yes/no, show your work, make a model/diagram, fill in the blank, solve, etc.
There is a total of 105 exit tickets:
Unit One: Place Value, Multiplication and Expressions (18 exit slips)
Unit Two: Division of Whole Numbers (9 exit slips)
Unit 3: Addition and Subtraction with Decimals (12 exit slips)
Units 4 & 5; Multiplication and Division with Decimals (16 exit slips)
Unit 6: Addition and Subtraction with Fractions (10 exit slips)
Units 7 & 8: Multiplication and Division with Fractions (15 exit tickets)
Unit 9: Algebra: Patterns and Graphing (7 tickets)
Unit 10: Convert Units of Measurement (7 tickets)
Unit 11: Geometry and Volume (11 tickets)

I taught a 5/6 combination class so these were a nice to have. Exit tickets are so important to help evaluate learning, but I found I was always forgetting to prepare them! These aligned with go math well. I only used one year because we changed curriculum.
