Classroom Library Checkout Forms

Students in a 1:1 environment can easily use their devices to check out books electronically with this easy to use Google Form in your classroom. Save some time setting up for Back to School season and download this pre-made, cute watercolor book header library checkout form.

What is it?

  • An electronic Google Form with unique fonts and custom watercolor book form header
  • This form links automatically to Google Sheets for easily filtering
  • Fields included on the form, which is EDITABLE include First name, Last name, Class period, Book Title, Author name, and a Class library terms of use acknowledgment

How to use it:

  • Can be shared with students via Google Classroom
  • Can be shared on Google Sites
  • Form responses are automatically filled out in a sheet within the inventory spreadsheet so you can keep track of which books are checked in or out, all in the same place
  • Directions are included in a Google Doc for how to filter the Google Sheet
  • A video tutorial is also linked for creating your own form and customizing the response sheet
