Description

I love to use this inventory at the beginning of the year to gauge how my students really feel about reading and to get an idea of what genre of literature circle novels to purchase.  With 20 questions, this thorough survey is perfect for teachers and paraprofessionals. This survey is appropriate for grades 4-8, and can easily be adjusted for high school. With this copy you will be able to tweak questions to get exactly the information you are looking for.  Assign this through Google Classroom, waste no paper, and let the form do the work. This form will automatically collate responses and show you a clear picture of what your students want to read with charts and graphs like these:

This Google Forms reading inventory is a fantastic digital tool for learning about your students’ reading habits, preferences, and attitudes. It’s easy to assign, quick for students to complete, and provides immediate, organized results that help guide independent reading choices and classroom instruction. I loved using it during the first week of school—it gave me a snapshot of each student as a reader without needing to sort through paper surveys. The questions are student-friendly and thoughtful, making it a great resource for building reading relationships right from the start. Thank you for offering this well-designed and time-saving tool for free — it’s a must-have for back-to-school planning!

Thank you for this beautiful resource. I would never downgrade anything over a simple typo, but I figured from one teacher to another, you'd love to know if something does have a typo in it. On page 3, you have "publec" instead of public, and on this question "If you struggle with reading, do you know why?" you have a lowercase "i" for if. I think that's the correct question. I changed it already on my master copy, so I can't be 100% certain if that's the correct one, but it was an "i" at the beginning of the sentence. Regardless, this is a wonderful form, and I am beyond grateful for you allowing us to use it. I hope you have a great day!
