Description

A Google Form is an effective tool to check understanding by the data collected or to use as a quiz assignment. This form practices graphing quadratic functions in intercept form. For each question ,students are asked to identify the coefficients, axis of symmetry, vertex, x-intercepts, and y-intercept. They will graph on a separate sheet of paper. The next question stays locked until they enter the correct answers.

Tips:

When asked to identify the coefficients in the order a, p, q these values come from the form:

y=a(x-p)(x-q)

  • 3 fall themed puzzles: how many scarecrows do you see, how many pairs of ghost eyes do you see, and unscramble the letters to make two words

Answers are included within the Google Form. The entire Google Form is editable.
