Sometimes taking a break on the bench isn't enough, keep these handy in the gym for when students behaviour goes too far, or if they have already been given several opportunities to correct the behaviour but fail to do so.

This can be used when sending a student out or if they need something more then just a break they can fill this in as their ticket to rejoin gym. Double check for accuracy and then send home for parents to sign. Students can answer questions in words and/or pictures depending on the age.
