This form is to be used to collect information from teachers regarding performance and progress to be included in student IEPs. The form is editable for you to customize to your school/district needs.
This resource can be used to gather uniform information from teachers and specific progress across subject areas. Sometimes obtaining information from general education teachers or multiple teachers who teach a student is difficult. With this form, you can email the link to all teachers to fill in.
Data obtained using this form can be automatically output into a google sheets spreadsheet which allows PLAAFP information from previous years to be stored and easily tracked from year to year.
The information collected from this form is the following:
1) Identifying information (Name of teacher, Name of Student, Month/Year of Report)
2) Present levels of functioning (in Math, Language Arts, Social Studies, Science and
Social Emotional Function)
3) Needs (in Math, Language Arts, Social Studies, Science and Social Emotional Function)
4) Strengths
*If teachers are unable to report in some areas, they can skip questions and still submit the information they have observed
