Description
This individualized behavior chart has worked great for me in my elementary art room. I use it like this: Teacher and student meet privately and discuss our MAIN goal for futures classes. Once we have agreed, we fill out name, goal, and date started. In each box I write a number, "1, 2, 3, 1, 2, 3..." Each class the student achieves their goal they can earn one sticker per class that is to cover up the number. Once they have earned "3" stickers they can choose a prize. Let me know if you have any questions! Enjoy!

Copied these on bright colored paper and students keep them on their desk. (I liked your suggestion of numbering the boxes). When they are finished or filled in, we send them home. :)
This was quick and easy to use in any setting. Thank you!
Very helpful!
