Its not worth for $3. Other than the name of students, only 10 questions with yes or no option.
Hi Aju, I’m sorry to hear you were disappointed. What I think is useful about this resource is that it will collate the results for you by student so you can better understand the unique learning styles or your students.
This activity worked out well with my students, they were able to follow along easily.

This is a great resource. The vocabulary for my ESL learners which is 90 percent was hard but the way the author wrote this, it was easy to understand. This worked as a quick in class activity to gauge my new students as I started with a new group middle of the year. I learned a lot quickly about their science habits. Thank you!
