This data sheet is one I created and used to collect data for a student on my caseload who eloped (left class suddenly and without permission) frequently, didn't complete work, was aggressive, and would be non-compliant toward most directions. On the other hand, it is important to know how many times the student is compliant and how long the student is on task. There is a column for each of these categories along with a place to write down who is documenting the data. There is also a place for you to put what the student is doing and where they are. For example, activity: art. Location: art room with Mr. Willis. So far, I have used this with 3 students. Administration loves the data to show the positives and negatives of the students' behavior.
