Description
This is the third in a series of 3 data collection templates. This sheet is to be used for intervention data only. There are 3 tabs in the document.
- The first sheet tracks how each student needing intervention did by question. There is conditional formatting for the percentage, but that will not be automatically calculated.
- The second sheet is Tier 1 intervention. Tier 1 intervention is for ALL students. This lets you record the most missed questions on your assessment. This data will need to be typed in or calculated in the sheet.
- The third sheet is for Tier 2 intervention. Tier 2 intervention is your small group that needs additional instruction from you. The data on here is pulled from the first sheet (you need to calculate the percentages) so you can see what to focus most of your time on. You can then provide individual students with targeted help on questions outside of the trend.
How to use:
- Type in student names
- Use codes to record accuracy
- Type in scores
The sheet is set to automatically calculate their percentage, which is color-coded by proficiency level.
All use conditional formatting on Google Sheets to make the data quick and easy to use. If you want to change colors or proficiency levels: highlight the column(s), click Format, select conditional formatting, and click the different colors to adjust the settings.
