This is a form that I have my students fill out if they are unable to turn in an assignment on the due date. It gives some responsibility to the student without punishing them for not turning an assignment in. Students have to fill out this form on the due date of the assignment and turn it in in lieu of the due assignment. They have a one day extension to turn in the assignment without getting a zero, missing assignment or whatever your homework policy is. The highlighted part of the form is for you to edit based on your homework policy.
