This is a template for entering your assignments and pt. values into your grade book. With Common Core (math especially) I don't have a textbook that gives me everything I need, so I pull a lot of things from several different sources. This is a way to keep everything sorted out.

In this template there is a place for the date, day of the week, assignment name and point value for that assignment. There is also a place for the Unit number and name and the number of school days in that unit. The preview file is an example of how I use this for my 6th grade math classes.

I use this to plan my year out, and even though I know the dates will probably have to be adjusted, it helps me see what kind of pace I need to keep in order to teach the concepts as fully as I need and want.
