This is a more detailed version of my Lesson Plan Template I. It was created in MS Word (tables) and you can add or delete any information you want. To change columns or rows, just put your cursor on the line and drag. You can also split cells or merge cells by right clicking in a cell. It also has a center schedule on page 2 for organizing guided reading centers. I run two small reading groups (Center #1 and #6) and then the other groups go to centers (#2, 3, 4, 5). Just copy onto legal size paper (8 1/2 by 14) either back to back or stapled.
