This document is a great way to easily organize student data. We all have students in our class who require additional academic and behavioral supports and this tool helps guide teachers through the beginning stages of intervening. More specifically, this product supports an easy to follow student background review, documents the intervention chosen to match student deficits, and a curriculum based measure menu of how you will be monitoring outcomes. This is a partial version of my MTSS Tiered Case Data Review and Script which then guides you through the documentation of progress as well as a script to use during team reviews and parent teacher conferences to showcase the students outcome results.
