I'm not sure about you, but at the end of each quarter/semester, and a the end of the school year, I like for my students to do an evaluation of my class so that I can improve the dynamics of my classroom. I have had great feedback from my students, and I have been able to take their comments, concerns and suggestions and implement them. If you think the students would be more honest without putting their names, you can simply tell them a name isn't necessary. I hope you enjoy!
