Instead of having several sheets of paper to file or hand back, why not keep all assessments together in a booklet. This booklet is simple to put together. You simply print double sided and staple. That's it! There are two different booklets. There is also rubric on the back for a quick reference when making groups, doing report cards, or for parent conferences.

Thank you for your feedback Bronwyn. I am sorry you were not completely satisfied with the Exit Slips. The intended use is for a quick assessment to see if the student understood the lesson rather than practicing the skill. I am open to suggestions on how I can improve the product.
